Annual Online Forms for Parents

Parents/Guardians,

Please log in to the CCPS Parent Portal (FOCUS) to review and complete the required annual student forms.

How to Access Online Forms

1. Log in to the CCPS Portal.
2. Click the drop-down arrow next to your name in the upper-right corner.
3. Select the form you need to review and complete. 

Forms to Review

1. Media Release Forms

Parents can review and submit:

• Media Release
• Directory Release
• Student Code of Conduct
• Student Laptop Agreement
• Programming Request
• Soda Agreement

To access these forms:

1. Log in to the CCPS Portal.
2. Click the drop-down arrow next to your name.
3. Select the appropriate form and submit. 

2. Emergency Information Form

Parents can update:

• Parent Contact Language
• Student Lives With Information
• Student Transportation
• Emergency Contacts

Note: Changes require school approval.

3. Health Information Form

Parents can update:

• Health Information
• Medical Conditions
• Required health notifications and consents

Submissions are automatically processed and reviewed by the school nurse. 

4. Name Deviation (Preferred Name) Form

Parents may request changes to a student's preferred name or nickname.

Important: Approval is at the discretion of the school administrator and requires administrator approval. 

5. Science (K-12) Safety Acknowledgement Form

Parents should:

• Review the safety rules with their student.
• Electronically sign the form.
• Submit the acknowledgement.

The form is automatically approved upon submission and can be viewed by the teacher.

Annual Review Required

All emergency, health, and science safety information forms must be reviewed and updated annually, even if no changes are needed. When finished, scroll to the bottom of the form and click Submit and Finish.
